NEW TRAVEL PERMIT PROCESS Use this form to request a travel permit. Ice events (exhibition games/tournaments) will entered on team calendar once a travel permit has been requested. ALL OMHA exhibition games and tournaments should NOT be considered approved until you have received a copy of the OMHA travel permit from the Town Contact. EXHIBITION GAMES scheduled as part of OWHA schedules are exempt

Travel Permits will take time to process and receive approval back from the OMHA.

Final registration into OMHA tournaments will NOT be complete until you have submitted your approved travel request form.

Submit this form at least 2 weeks prior to the tournament or exhibition game.

I understand a travel permit for a tournament or exhibition games are not granted until OMHA paperwork has been received^*
