$35 charge on all NSF cheques

CASH CALENDAR TICKET FUNDRAISER:

Each family is required to participate in TCDMHA Cash Calendar Ticket fundraiser. Each player receives $150 worth of tickets to sell (maximum $300 per family). Tickets are given out at registration and a post dated cheque is required dated OCTOBER 31, 2012
FAMILY RATE: Oldest Child & 2^nd Child — Full Registration; 3^rd Child — 25% off Registration; 4^th & Each Additional Child — 50% off Registration

REP FEE:

All players on rep teams are charged an additional $ 25.00 fee above registration to cover the additional costs related to rep teams — team registration fees and OMHA playoffs fees. This fee will be due after team selection and before first practice.

INTERMEDIATE GIRLS/JUVENILE BOYS
Registration Cheques and Post Dated cheque for Cash Calendar will be received during registration but no Cash Calendar tickets will be released until team is confirmed.

No Player Movement or Permission to Skate will be issued unless registered prior to June 1st 2012.
